Use this form to list the medications you are currently taking, including as needed medications, over the counter medications, herbal medications and vitamins. Bring this list with you to your next appointment. For your own safety, carry a current list of medications with you at all times. We will be asking you to verify the medications you are taking when you come to the Emergency & Trauma Center and any scheduled visits.
