Billings Clinic Orthopedics & Sports Medicine designates two three-day sessions each year for eastern Montana schools to provide sports physicals for all athletic activities.

Please download and complete this form, Pre-Participation Physical Exam Form, and bring it with you to your sports physical.

As a community service, the sports physicals are significantly discounted and the majority of the funds collected are donated back to the schools.
