Geriatric Primary Care

We offer primary care for the older adult. This includes office visits, health screening, and care for the chronic conditions you may have – with an emphasis on your stage in life.

Geriatric Assessment Program

The goal of the program is to evaluate and assess cognitive, emotional, and functional capabilities. It is a comprehensive evaluation which includes a detailed history, thorough exam, and coordination with the family and primary care provider. The assessment includes screening for geriatric syndromes such as falls, gait impairment, sleep problems, incontinence, depression, dementia, frailty, malnutrition, and behavioral problems which occur with some of these syndromes. We can connect patients and families with community resources to help meet the needs and goals of our patients. As well, we can provide helpful suggestions for handling many of the problems faced by the patient, their family, and caregivers.
