Rapid Response Team and Condition HELP

Your Safety Matters

We believe families play an important role in patient care. You know your loved one best, and we want you to feel comfortable sharing concerns and asking questions whenever something doesn't seem right.

Your safety and well-being are our highest priorities. If you or your loved one experiences a sudden change in condition while in the hospital, additional support is available through our Rapid Response Team (RRT). We also encourage patients and families to speak up if they have concerns through a program called Condition Help.

What Is the Rapid Response Team?

The Rapid Response Team is a specialized group of healthcare professionals who can quickly respond when a patient needs urgent medical attention. The team works alongside the patient's care team to provide additional assessment, support, and treatment when needed.

A Rapid Response Team may become involved if:

  • You or your loved one experiences a sudden change in health.
  • There are concerns about breathing, blood pressure, or other vital signs.
  • A patient's condition appears to be getting worse.
  • Additional medical expertise is needed to evaluate the situation.
  • A higher level of care may be required.

Our goal is simple: identify concerns early and provide the right care as quickly as possible.

What Is Condition HELP?

Condition HELP is designed to give patients and families an extra way to ask for assistance when they are concerned about a patient's condition. If you feel that you or your loved one's health concerns have not been fully addressed, you can request an additional clinical evaluation.

Condition Help empowers you to be an active part of the care team. Speaking up about concerns can help ensure that important changes or symptoms receive the attention they deserve.

When Should I Request Condition Help?

You may want to request Condition HELP if:

  • You notice a significant change in your loved one's condition.
  • You are worried that a patient is becoming sicker.
  • You have spoken with the care team and still feel additional evaluation is needed.
  • You would like another clinical review because something doesn't seem right.

We encourage you to share any concerns you may have. Your voice is important, and we want you to feel comfortable speaking up whenever something doesn't seem right.

What Should I Do First?

If you have concerns about you or your loved one's condition, we encourage you to:

  1. Speak with your nurse or healthcare provider.
  2. Ask to speak with the charge nurse if your concerns continue.
  3. Request Condition Help if you feel additional assistance is needed.

Our care teams are committed to listening, answering questions, and partnering with patients and families.
